Endometriosis is a disorder of deranged immunity that devastates the reproductive potential and affects the psychosexual lives of countless couples. A very high recurrence rate requires lifelong surveillance. It presents in myriad ways and often may silently progress for many years prior to a definitive diagnosis. There is a dire need to develop a biomarker that will diagnose its early presence. When diagnosed in late stages, both medical and surgical management can be challenging, and most often both have to be combined. Surgical management requires considerable expertise in endoscopy to perform total disease excision without major organ damage. Adding effective medical management drugs both prior to and post-surgery will ease the surgical procedure and prevent recurrence. This case series is an endeavour to bring video learning and practical case management in the most challenging situations. The editors hope that it will be useful for clinicians and that countless patients will benefit from a combined medical and surgical tailored approach.
